CA Privacy Notice

Clove Dental – California Employee Privacy Notice

The California Consumer Privacy Act of 2018 (“CCPA”), as amended, requires Clove Dental (“Clove,” “we,” “us,” or “our”) to provide notice to its job applicants, employees, and contractors who are California residents (“California Persons”) regarding the categories of personal information that may be collected and the purposes for which that information is used.

This California Employee Privacy Notice (“Notice”) is intended to satisfy the CCPA’s notice requirement and applies solely to California Persons in their capacity as employees, contractors, and job applicants.

1. Updates to This Notice

We may update this Notice periodically—at least once every twelve (12) months—to reflect changes in our practices, legal obligations, or operations. The most current version will be posted at https://clovedds.com.

We will not collect additional categories of personal information or use personal information for materially different purposes without providing prior notice.

2. Definitions

  • “Employees,” “employee,” or “you”: Includes current and former full-time, part-time, temporary, and contract workers, directors, officers, and job applicants.

  • “Personal information”: Information that identifies, relates to, describes, or could reasonably be linked with a particular individual or household. Publicly available information and de-identified data are not considered personal information under the CCPA.

3. Categories of Personal Information Collected

We may collect the following categories of personal information about California Persons:

  • Identifiers: Name, address, phone number, email, social security number, driver's license, passport, tax ID, NPI, etc.

  • Commercial information: Vehicle details, license plate, VIN.

  • Device and Online Identifiers: IP address, MAC address, device ID, browsing history.

  • Biometric Information: Photos, height, weight, shirt size.

  • Geolocation Data: GPS or device-based location data.

  • Sensory Data: Video surveillance, voice recordings, photographs.

  • Professional/Employment Information: Job history, salary, benefits, work status, performance, training, certifications, licenses, claims, disciplinary records.

  • Education Information: Degrees, transcripts, certifications, and training.

  • Characteristics of protected classifications: Age, race, gender, citizenship, disability status, marital status, veteran status, health data.

  • Financial & Legal Data: Bank account, insurance numbers, garnishment records, subpoenas.

4. Sources of Personal Information

We collect personal information:

  • Directly from you (e.g., job applications, onboarding documents).

  • Indirectly from you (e.g., via our websites, systems, and devices).

  • From third parties (e.g., background check providers, recruiters).

5. How We Use Personal Information

We may use the categories of personal information listed above for the following purposes:

  • Recruitment and onboarding

  • Credentialing healthcare professionals

  • Administering payroll and benefits

  • Performance evaluations and career planning

  • Monitoring professional licenses and compliance

  • Human resources operations and employee records

  • Emergency contact and safety

  • Eligibility to work verification

  • Medical screenings and workplace health services

  • Workplace training and engagement

  • Security and IT operations

  • Legal and regulatory compliance

Personal information may also be used as reasonably necessary to achieve these purposes or as required or permitted by law.

6. Your Rights and Access

While employees currently do not have full access or deletion rights under the CCPA, Clove Dental is committed to transparency and allows personnel to access and update certain personal data as required under applicable laws. Contact your People Services Representative for support.

Monitoring and Consent:
Your use of Clove Dental systems (email, communication, and IT resources) is not private and may be monitored for legal compliance, security, and operational purposes.

7. SMS Opt-In and Marketing Communication

Clove Dental does not share any SMS opt-in or mobile message consent information with third parties or affiliates for marketing purposes.

SMS data is collected solely for operational, service-related, or emergency communication purposes. Consent is required prior to sending text messages, and users may opt out at any time.
